Lewisham Council on behalf of Lewisham Clinical Commissioning Group are looking to procure a Primary Care Dietetics Service for adults in Lewisham.Lewisham seek to commission an experienced and suitably qualified organisation to deliver a primary care dietetic service within Lewisham. The commissioned service will consist of the following three interlinked components delivered by one provider:1. Continuation of an existing service that comprises of:a) practice-based dietetic service with a focus on clinical conditions requiring specialist dietetic intervention, delivered in all 41 GP practices in Lewisham;b) borough-wide specialist dietetic weight management service as part of the community tiered adult weight management pathway.2. A new dietetic provision to Nursing Homes in Lewisham.This will provide a service to nursing homes including Learning Disability and Mental Health care homes to help reduce the incidence of malnutrition. The dietetic intervention will include training and individual client assessment.3. A new dietetic provision for patients discharged on oral nutrition support from secondary care.This element of the service will work closely with the existing Lewisham Integrated Medicines Optimisation Service (LIMOS) team. The service will oversee, monitor and review oral nutrition supplement prescriptions.
